PLANOUTCOMECRITERIA

Patientwillverbalizeknownfearsattheendofshift.

Patientwillverbalizeanddemonstrateeffectivecopingmechanismbythetimeofdischarge.

INTERVENTIONS

Thenursewillspendtimewiththepatientdiscussingfearsandallowthepatienttoexpressfears.

RNwillinstructthepatientonrelaxationtechniquesincludingbiofeedback,yoga,andimagery.

RNwillprovidedistractioninterventionswhenpatientisfeelingincreasedfear.

Rational

Allowingthepatienttoopenlydiscussfearwillhelpthepatienttogetintotouchwiththeirfearsandtobetterunderstandtheirwhattriggerstheirfears.

Relaxationtechniqueshelptoreducefearandanxiety.Thesetechniqueswillaidthepatientindevelopingcopingstrategiesthatcanbeusedwhenontheirownanddischargedfromthehospital.

Distractioncanaidinacutefearandanxietytodivertattentionfromthefear.

EVALUATION

Ptdemonstratesoptimismforgoinghomebutalsoexpressesfearaboutcaringforselfandfamilyoncehome.
